Personnel Files Are the Most Common Weak Spot
Missing license verifications, expired background checks, incomplete orientation checklists, and lapsed CPR or competency documentation generate findings in nearly every survey. Create a credentialing calendar that flags expirations 60 and 30 days out, and audit every personnel file before the inspector ever walks in.
Train Staff to Answer Questions Confidently
Surveyors interview direct care staff, not just leadership. Questions like “What do you do if a client falls?” or “How do you report a change in condition?” are answered well only when staff were genuinely trained and recently. Run refresher trainings and quick-fire practice sessions in the weeks before an inspection. A staff member who says “I’m not sure” creates a finding; one who walks you through the policy does not.
Walk Your Own Environment
Physical environment citations are among the easiest to prevent: blocked fire exits, unlabeled chemicals, missing first-aid supplies, improperly stored medications, and absent emergency contact postings. Do a documented self-inspection walkthrough using your state’s checklist, correct everything, and keep the record. Documented self-audits impress surveyors and demonstrate an active quality program.
Have Your Incident and QAPI Records Ready
Inspectors will ask how you handle incidents, complaints, and infections, and they will want evidence of your quality assurance program: meeting minutes, trend reviews, and corrective actions. An empty incident log looks worse than one with resolved, documented incidents. Maintain your QAPI rhythm and keep minutes current.
